Hi, here is the instrument we used during my senior internship as for our paper.  During that time, we really did not do much of an analysis of it's reliability and validity and just jumped into translating it to validate it for future use in Filipino studies on dry eyes. This was the instrument used by a lot of our residents since it is open access and has a fair number of validation studies and has been translated in several languages (Spanish, Chinese) but not in Filipino (Tagalog).
