Thanks for this response. Your example of lung transplantation in general, and survival in patients with CF more specifically, are very relevant one for the socioecological model. For people with CF, it would be relevant to consider health care disparities, which we will discuss in two weeks - and particularly to what extent differences in care could be due to differences in the systems in which people receive care, as opposed to differences in how different demographics of people are treated within systems. We will discuss this in some depth. As you point out, there are also other relevant layers of the SEM, including access to adequate housing, transportation, health literacy and nutrition, which could also play a role. 

The example of lung transplantation also raises an issue about how to define health disparities, vs. process outcomes on the path to health disparities. I would consider listing for lung transplantation a process outcome, or could also be considered more directly a health care disparity. The actual health disparity itself would be death or morbidity from lung disease. I know this may seem semantic, but as the language about disparities can get muddled it is helpful to have this clear in your head.
