As you mentioned above, I think the role of SES in the relationship you describe in question 2 (race/ethnicity and development of NAFLD) is quite important. It might be interesting to look at other factors like median income by zip code or education as potential effect modifiers (not sure if this is in the database you plan to use, but it is included in the UNOS data).
