I agree with your implications for future research. The interrater reliability could be a challenge given the potential interpretation of some of the scale items, as you mentioned. Consistent scorers and clear training should be helpful to resolve these issues. To some degree, future studies will likely have more consistency than the range of interpretations one might see in the community in clinical use, but at least the bar will be appropriately held more highly for study purposes.
