As you point out, the stigma around talking about incontinence and sexual dysfunction is an important consideration that can interact with the other structural and interpersonal causes of health care disparities. I think this points out that while there are many common pathways by which health care disparities can occur, there is also specific considerations that are more or less relevant to specific areas of health care.
