All of your examples of structural issues are right on target – I think your example of the challenge of child care is one that is often not recognized. Getting to a doctor's visit is hard, and having a clinic that does not facilitate patients with multiple competing demands being able to get there will inherently have health care disparities. I myself have inserted an IUD for a woman as she held her baby on her chest, because she had no one to take care of it while she was having the procedure – which was making the best of the situation, but for many women is not what they would choose. And I definitely agree that the issue of diversity in the provider workforce is a systems issue, which then leads to the interpersonal issues that we discussed in class.  And I love your idea of an implicit bias intervention – this is something that I think we need to work towards integrating into medical education, combined with a broader, structural understanding of social determinants.
