    How to request a new "Guest” account (for anyone who has never had a “guest” account previously): 

    Please send     Frances Lee (Frances.Lee@ucsf.edu) the following information for each individual: 
    • Full Legal Name, 
    • SS#, 
    • Email, 
    • DOB, 
    • Begin and End Dates, 
    • Country, 
    • Reason for Request
    Once Frances processes the request in the system, individual will receive an email with instruction to complete the process within 7 days.
