As someone who studies rural health, which is so often thought of in terms of distance/remoteness, I'm very intrigued by your conception of proximity as an effect modifier. It makes intuitive sense, but I don't know if I would have ever thought of distance/proximity in those terms. I'm curious now to see how this approach has been applied in the rural health literature.
