QUESTIONNAIRES AND QUALITATIVE RESEARCH
(Note that you will incorporate material for this assignment into the Final Protocol that you will submit by Friday in your fourth week. Incorporate this into a combined protocol document that includes the material you have developed and refined for other assignments.)

Almost all studies use at least one questionnaire to collect data on demographic variables, health-related habits (such as smoking or alcohol consumption), medical history, or variables such as depression score, cognitive function score, dietary intake, etc. Choose the most important questionnaire that you will use in your study (or, if you are analyzing data from an existing dataset, choose a questionnaire that was administered as part of the original study):
- Briefly review the literature on the characteristics of this questionnaire. What domains are assessed? How has the questionnaire been validated; if so, in which populations?
- What other considerations may be important for using the questionnaire in your study (length, administration, translation, etc.)? Provide a brief justification for your focus on this measure, compared to other options for assessing the same construct.
- Place a copy of the questionnaire you selected in your Protocol Appendix.
