You asked about where you get published utility estimates. Attached is a 2006 paper by Sullivan and Ghushchyan in Medical Decision Making that includes utilities for a wide range of chronic health conditions.
You can also try the Tufts CEA Registry at http://healtheconomicsdev.tuftsmedicalcenter.org/cear2/search/search.aspx
The problem with this registry is that it appears that you can only-y access the full data base with a ‘premium’ account.
Perhaps the best approach is to search the literature for other CUAs on the condition of interest, and see what other analysts used and where they got their utility estimates.
