Right now, my project is in the Formative Evaluation phase (qualitative interviews to determine what’s necessary to carry out the program in accord with the desired goals and objectives)

From there an intervention will be developed, and process, intermediate outcome and outcome indicators will be more explicit.  For the sake of the exercise, I will assume that the intervention is an in-clinic NP that provides reproductive healthcare for all teens of reproductive age (as part of multidisciplinary rheumatology clinic)

Process Indicators:

  • Have all patients that are post-menarchal or >12y/o females on teratogenic medications been identified? (EHR review)
  • Have all patients that are post-menarchal or >12y/o females on teratogenic medications participated in the multidisciplinary clinic? (EHR review)

Outcome indicators (for improved provision of reproductive healthcare):

  • Have all patients that participated in NP reproductive health care clinics been counseled re: teratogenic risks of their medications? (EHR)
  • Was contraceptive counseling provided for all of these patients? (EHR)
  • Were urine pregnancy tests sent on all patients newly started on teratogenic medications? (EHR review)
  • Were patients more aware of teratogenic risks of their medication (pre and post intervention knowledge, attitudes and beliefs survey)
  • improved prescription rates of contraception (and continuation of contraception)

Intermediate Outcome measures:

  • Has this multidisciplinary clinic been widely accepted by parents, patients and physicians? (survey or brief qualitative interviews)

Impact Indicators:

  • Decreased incidence of unintended pregnancy in this population

Your impact indicator is great as the larger goal.  For this project and the type of access to information you have, you might frame it as "Reduction of pregnancies while actively taking teratogenic medicines" since it might be hard for you to determine "unintended" or not.

Very thorough, clear description, including how you would access the information. I like the intermediate outcome, because I know this can be a tricky program to introduce in a pediatric population. An additional process indicator could be assessing the  training of rheumatology providers and staff about the contraceptive counseling program
