I like the tabular format with “Means of Verification” columns! You’ve really drilled down into the very basics of how this program is also part of a research protocol. That’s great. I also like the way you’ve adapted this to the logic model terms of Inputs (Processes) and Outputs (outcomes and impact).
Your intermediate outcomes are also good. This is where you get to evaluate, for your program, if the tools/activities improved knowledge, attitudes, skills, self-efficacy and behavior. You might at some point also add a patient survey/assessment component to your evaluation.
