E.  The authors' conclusion was, "Among patients with type 2 diabetes with or without previous cardiovascular disease, the incidence of major adverse cardiovascular events did not differ significantly between patients who received exenatide and those who received placebo."  Is this conclusion consistent with the 95% CI for the hazard ratio they reported for their primary outcome? 

Here, are we assuming that by "major adverse cardiovascular events" the authors are referring to their primary composite outcome (i.e., death from CV causes, nonfatal MI, nonfatal stroke)? Or does "major adverse cardiovascular events" not include stroke?

Yes, by “major cardiovascular events”, the authors are referring to their primary composite outcome.
