The trial was a randomized trial of invitation to screening, not of screening itself. The intention-to-treat analysis provides the effect of invitation to screening. But we want to know the effect of screening itself (not invitation to screening) on death from any cause. (Hint: See PSet #9, Problem #1 on the Multicenter Aneurysm Screening Study.)
