Quick clarification for question 6d: When you say "Assume that you would be willing to do 200 tissue biopsies to avoid missing a necrotizing fasciitis diagnosis" do you mean 200 unneccessary biopsies?

No, I meant 200 biopsies. I am not distinguishing between necessary and unnecessary. (I'm not sure a negative biopsy would not necessarily be unnecessary.)
Would another way of phrasing this be to say that you would be willing to do 200 biopsies even if only 1 demonstrated nec fasc? (and 199 did not?)
