Hi - I also had trouble figuring out this homework, and getting a feel for what type of data we're trying to collect.
In terms of a framework, your focus on provider barriers could fit well with the COM-B model. Several of your reasons for not referring patients have to do with motivation. The diffusion of innovations framework could also have some insights. What do you see with the early adopters of self-testing? Are they successful? Are they people who are likely to be emulated by their colleagues?
