Thanks for your effort to solve an important implementation gap in the HIV health system in Sierra Leone. 

When you mention uptake of the Option B+ protocol as the implementation gap, I want to suggest that you consider where this issue is situated in the cascade of HIV care. You may want to focus on initiation of ART and subsequent health outcomes in this population. Are there any baseline data describing the cascade of care for Option B+ in Sierra Leone? Also, I would like to learn more about the uptake issues among healthcare workers in Sierra Leone. If Nevarapine is sitting in the L&D ward, why isn't it being initiated?  I believe this PMTCT Option B+ occurred on a global scale. It would be interesting to understand if uptake of the protocol was an issue in other countries and how addressing this issue in Sierra Leone might add value to the field of implementation science.

Hello, this topic of prevention of MTCT in Sierra Leone is great case for delving into why an intervention with great uptake in one setting may not roll out as smoothly in another. You note that access to medication seems to have been addressed in Sierra Leone and you identify health care workers as a possible roadblock to successful implementation - are there specific barriers faced by health care workers in Sierra Leone that an implementation project might target?
