    • Ambulatory AP/GYN Evaluations:

    • Weekly

    • Clinical Goal Setting Form File
      One page form for the entire quarter to complete at each session with preceptor

    • 2-3 Times a Quarter

    • Formative Evaluation File

      This is an Ad Hoc Evaluation sent by you to you preceptor and to be completed by your preceptor at times designated by the FOR. It will be 2-3 times a quarter.

    • At the end of Each Quarter:

    • Preceptor Evaluation File

      To be completed quarterly. The clinical course FOR reviews evaluations quarterly and the student names is visible to them on the evaluation. Evaluations will only be shared with the preceptor in an aggregated format, when three or more evaluations are available. Student names are not tied to the aggregated data.


    • At the End of Each Course:

    • Summative Clinical Evaluation Form File

      To be completed by the FOR at the end of the course. Evaluations will be based on formative evaluations as well as FOR communication with preceptors and students.
